CNA - Home Health, Hospice, Sunshine Cares
Brief Summary
A qualified CNA Candidate will be able to provide personal care across three distinct environments. As a Home Health, Hospice, or Sunshine Cares CNA, the primary focus of this position is to provide companionship and personalized care while encouraging independence and self-care. The CNA will assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating. The CNA will provide mobility and transfer assistance, which may include using mechanical lift devices. The CNA will also Prepare meals and snacks.
Professional Breakdown of CNA Responsibilities

• 
  
• Follows the patient's service plan when providing the required care. Conducts job responsibilities in accordance with company standards, policies, procedures, and in accordance with the state and federal regulations.
  
• Reports directly to the CNA Coordinator for scheduling and logistical details. Reports directly to the RN Case Manager or Clinical Director on any patient care details.
  
• Must be able to work independently, make judgements based on assessments and data available and act accordingly. Must be flexible, innovative, and possess mature interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
  
• Under the direction and ongoing supervision of the RN Case Manager, the CNA will provide essential personal care assistance to the patient including:
  
  • 
    
  • Bathing, care of mouth, skin, and hair.
    
  • Assisting with bathroom or in the use of a bedpan or commode.
    
  • Performs bed transfers and ambulation activities (assistance to walk) as ordered by the care plan.
    
  • Makes daily entries into patient records.
    
    
    
  • Assists with eating and meal prep, including meals with a specific diet. Monitors food and fluid intake as specified by the patient's care plan.
    
  • Assists patients with preparing for bed. This includes the changing of linens.
    
  • Takes and records a patient's temperature, blood pressure, pulse, and respiration rates.
    
  • Documents and reports the observations of a patient's behavior, complaints, physical symptoms or other changes to RN Case Manager. Documentation and reporting is to be accomplished in a timely manner.
    
  • Attends departmental meetings, briefings, and all training sessions.
    
  • Follows all safety standards related to infection control, universal precautions, and OSHA standards for healthcare professionals.
    
  • Maintains a positive and collaborative relationship with the patient, the patient's family, and coworkers.
    
  • May assist in light housekeeping.
    
  • May assist with prescribed exercises as designated by a Physical Therapist.
    
    Job Requirements

    • Education- Highschool Diploma or equivalent.
      
    • Certification-
      
      • 
        
      • Not required at time of hire. Finance Assistance to become a Certified Nursing Assistant is available. Achievement of Certification is encouraged.
        
      • CPR and First Aide Certification is required at a point in time after hire.
        
      • Must have a valid driver's license, proof of auto insurance, and reliable transportation.
