Aegis Home Health is currently seeking a compassionate Physical Therapist that has a passion for providing outstanding services to the elderly in Senior Living Communities and in-home health settings. This is a full-time position serving patients in Cache and Box Elder counties.
Each Physical Therapist is an important part of the Aegis Health Care team. Aegis specializes in providing care for home health patients in home settings and in senior living communities.           Job Responsibilities:
          
          • 
            
          • Provide physical therapy treatment and patient care according to state and federal regulations and professional guidelines
            
          • Perform complete evaluations of the patients' functional capacity, muscle strength, motor development, respiratory and circulatory efficiency, and endurance
            
          • Routinely assess and revise therapy care plan to ensure best patient outcome
            
          • Provide education and instruction to patients and family members regarding therapeutic exercises to facilitate self-care within the limits of patient's abilities
            
          • Delegate tasks and supervise physical therapist assistants and technicians
            
          • Maintain open and ongoing communication with peers to ensure patient needs are met
            
          • Accurate and timely documentation of observations and evaluations
            
            Skills & Qualifications:
            
            • 
              
            • Current Physical Therapy License in the state of practice
              
            • Home Health experience preferred
              
            • Ability to work independently
              
            • Time management skills
              
            • Effective communication skills, both verbal and written
              
            • Valid driver's license, clean driving record and reliable automobile
              
            • Proof of current automobile insurance
              
            • Criminal background check clearance/fingerprint clearance
              
            • Current CPR certification
              
            • Negative TB test
