Aegis Home Health and Hospice is currently seeking a compassionate Registered Nurse (RN) that has a passion for providing outstanding services to the elderly in senior living communities and in-home health settings. This is a full-time position serving Cache and Box Elder counties.
Each Registered Nurse is an important part of the Aegis Health Care team. Aegis specializes in providing care for hospice and home health patients, including respite care and continuous care in senior living communities or in the patient's home.           Job Responsibilities:
          
          • 
            
          • Perform appropriate assessment of physical, social, and psychological status
            
          • Use assessment information and critical thinking skills to develop an individualized plan of care; revise and reassess changes as appropriate
            
          • Actively seek patient, family, team, and physician involvement to develop plan of care including needed education
            
          • Set priorities, coordinate, and organize patient care delivery through effectively managing time, supplies, and resources
            
          • Initiate referrals as needed to other health team members
            
          • Maintain patient records showing systematic assessment, planning intervention, and evaluation
            
          • Participate in team meetings and coordinate patient care with other members of the team
            
            Skills and Qualifications:
            
            • 
              
            • Current Registered Nurse (RN) license in state of practice
              
            • Home Health / Hospice experience as a Registered Nurse (RN) preferred
              
            • Ability to utilize full range of movement to transfer patients
              
            • Ability to communicate effectively both verbally and in writing
              
            • Valid Driver's license, clean driving record, and reliable automobile
              
            • Proof of current automobile insurance
              
            • Criminal background check clearance/fingerprint clearance
              
            • Current CPR certification
              
            • Negative TB test
